Insurance Information – Medical Conditions

Our practice accepts the following insurance plans for the evaluation and treatment of medical conditions.

Blue Cross Blue Shield
GHI Emblem Health
NYS Empire Plan
Local 1199
United Healthcare
UPN Elite

Please call (917) 522-1825 for insurance verification and to learn more about which physicians take new patients for each insurance plan.

Payment for any deductibles and co-payments is due at the time of service. Health insurance plans do not cover procedures that are not medically necessary, which includes all cosmetic procedures.

Securing referrals is the responsibility of the patient. If a claim is denied due to an invalid, incorrect, or missing referral, the patient is responsible for the cost of the visit.

Insurance plans vary on their coverage and may provide only partial reimbursement of our fees. It is important to understand what your insurance plan is. If we do not participate with your insurance plan but your plan has “out of network” benefits, we are happy to provide the necessary documentation for submitting an “out of network” claim. If we do not participate with your plan, you will responsible for full payment at the time of service.
